Chapter 432 — Vital Statistics
ORS 432.148 Commemorative Certificate of Stillbirth; rules
The State Registrar of the Center for Health Statistics shall establish a Commemorative Certificate of Stillbirth. The certificate shall be signed by the state registrar.
The state registrar shall issue a Commemorative Certificate of Stillbirth for a stillbirth occurring on or after January 1, 1999, upon:
Request of a genetic parent of the stillborn fetus; and
Payment of the fee adopted by the state registrar by rule.
The state registrar shall adopt by rule:
A form for the certificate;
The type of information that may be included on the form; and
The fee required for issuance of the certificate.
A certificate issued under this section is for commemorative purposes only and has no legal effect.
